Q: Is Vasona Creek Healthcare Center a Medicare and Medical certified skilled nursing facility?
A: We are a 24-hour skilled nursing and rehabilitation facility. In addition to Medicare, Medi-Cal and Private Pay, we are contracted with Kaiser, United Healthcare, Blue Cross, Healthnet, Santa Clara Health Plan and Cigna.
Q: How many days per week do residents receive therapy?
A: All residents will be assessed by our Rehab team and a personalized plan of rehab care will be created in conjunction with the attending physician to facilitate returning to the highest level of functioning. Rehab services are provided 7 days a week.
Q: What are the facility’s visiting hours?
A: We don’t have standard visiting hours. We recommend visiting between the hours of 9:00 a.m. to 7:00 p.m.
Q: How is residents’ laundry done?
A: Our facility offers laundry services, but family members are welcome to do laundry at home if they wish.
Q: Who makes transportation arrangements for residents’ doctor appointments?
A: Our staff will assist you with your transportations needs.
Q: Do you allow pets to visit the facility?
A: Pets are welcome in our facility as long as they are vaccinated and an updated record can be shown at the receptionist’s desk or nursing stations.
Q: May I take a resident out of the facility?
A: Vasona Creek Experience Residents may be taken out of the facility only if there is a physician's written order. Residents need to be signed out by their responsible party at the nurses’ station.
