The Social Services Department at Vasona Creek Healthcare Center plays a crucial role in coordinating a safe discharge plan for our patients who are receiving short-term nursing care and therapy services. To ensure a smooth transition home, the social services team works alongside physicians and extended departments such as nursing, rehab and dietary. We consult multiple experts to keep our focus on accommodating each individual patient's needs.
We offer services to provide support and assistance to our patients and family members as it relates to clinical care and psychosocial well-being. We strive to maintain open communication with those whom we serve to keep them well informed on the progress and plan of care.
